After our first phone call, we will send you an email with a link to download our patient portal app. You can fill out Patient Intake Forms and check detailed information of initial evaluation. The initial evaluation will be 60 minutes where we will discuss your injury, current activity level, and future goals. Your PT will go through a full body evaluation which will include strength, range of motion, functional movement patterns, and performance tests if applicable. This will help us make and initiate your plan of care moving forward.
Please wear comfortable clothing you can move in, bring indoor exercise shoes, and water bottles(we also provide water bottles in case you need it). If you have any images, referrals, or prescriptions given to you by your doctor, bring it when you have the initial evaluation.

No, since 2014, California has allowed direct access to physical therapy, meaning patients can see a physical therapist without a referral from a doctor or other healthcare provider. If the treatment lasts longer than 45 days or 12 visits, a dated signature on the plan of care from your MD will be required. We can fax your plan of care to your doctor.
Elite PT and Performance is an Out of Network Provider. Unfortunately insurance companies do not let us work with clients the way we have found to be the most successful. Cash-based physical therapy can be a better option for some patients because of personalized care(1 on 1 with licensed PT for 60 mins), potentially faster recovery times, flexible scheduling, freedom to choose therapists, no visit limits, and potential cost savings.
Many of our clients do get reimbursed a certain percentage of your PT services if you have a PPO. We recommend calling your insurance company to verify your Out-of-Network Benefits. We can help you navigate this.
Some patients also utilize their HSA and/or FSA.
Yes, we currently have a number of clients with the SDP program via PPL, GT, or Alta. Clients with SDP funds were allowed to have more control over their services and support, including the flexibility to choose and pay for providers. If you work with a Financial Management Services (FMS), they will handle the financial aspects of your plan. We will provide all necessary documentation.
A 24-hour notice is required for cancellations. Failure to provide 24-hour notice will result in a $50 late cancellation fee. In cases of emergencies or unavoidable circumstances, the fee may be waived on a case-by-case basis.
